"Leishmania no Brasil"

A Leishmaniose visceral canina é uma doença grave que acomete vários mamíferos, transmitida por um protozoário que tem o nome científico de Leishmania chagasi (infantum). O seu principal transmissor (vetor) é um inseto (flebotomíneo), da espécie Lutzomyia longipalpis, também conhecido como “mosquito palha”. O contágio em cães e no homem ocorre através da picada do inseto infectado.

O cão é considerado um importante reservatório do parasita pela sua proximidade com o homem e constitui o principal elo na cadeia de transmissão de Leishmaniose visceral nas zonas urbanas. Há outros animais silvestres que podem servir de hospedeiros intermediários desta doença, mas é impossível pegar a doença por contato direto com esses animais.

A Leishmaniose não é transmitida através de lambidas, mordidas ou afagos. O contágio ocorre somente através da picada da fêmea infectada do “mosquito palha”.

Targeting the neurokinin receptor 1 with aprepitant: a novel antipruritic strategy.

Abstract

BACKGROUND: Chronic pruritus is a global clinical problem with a high impact on the quality of life and lack of specific therapies. It is an excruciating and frequent symptom of e.g. uncurable renal, liver and skin diseases which often does not respond to conventional treatment with e.g. antihistamines. Therefore antipruritic therapies which target physiological mechanisms of pruritus need to be developed. Substance P (SP) is a major mediator of pruritus. As it binds to the neurokinin receptor 1 (NKR1), we evaluated if the application of a NKR1 antagonist would significantly decrease chronic pruritus.

METHODS AND FINDINGS: Twenty hitherto untreatable patients with chronic pruritus (12 female, 8 male; mean age, 66.7 years) were treated with the NKR1 antagonist aprepitant 80 mg for one week. 16 of 20 patients (80%) experienced a considerable reduction of itch intensity, as assessed by the visual analog scale (VAS, range 0 to 10). Considering all patients, the mean value of pruritus intensity was significantly reduced from 8.4 VAS points (SD ±1.7) before treatment to 4.9 VAS points (SD ±3.2) (p<0.001, CI 1.913-5.187). Patients with dermatological diseases (e.g. atopic diathesis, prurigo nodularis) had the best profit from the treatment. Side-effects were mild (nausea, vertigo, and drowsiness) and only occurred in three patients.

CONCLUSIONS: The high response rate in patients with therapy refractory pruritus suggests that the NKR1 antagonist aprepitant may indeed exhibit antipruritic effects and may present a novel, effective treatment strategy based on pathophysiology of chronic pruritus. The results are promising enough to warrant confirming the efficacy of NKR1 antagonists in a randomized, controlled clinical trial.

O CEDV, teve concluída sua terceira edição (2009-2010), congregando 24 inscritos, provindos de distintos municípios paulistas (Piracicaba, São Bernardo do Campo, São Caetano do Sul, Araras, São José dos Campos, Jacareí, Suzano, Americana, Arujá, Indaiatuba, São Paulo), fluminense (Niterói), capixaba (Vitória) e goiano (Distrito Federal – Brasília), que cumpriram em sete módulos, mais de 560 horas atividades. Trata-se do pioneiro curso de especialização no contexto latino americano e o único, até hoje, homologado pela Sociedade Brasileira de Dermatologia Veterinária. Com a conclusão deste terceiro módulo ultrapassa-se o nº de 70 egressos habilitados militantes, na área de dermatologia veterinária, nos Estados de SP, PR, MG, RJ, SC, RS, GO, (DF), ES.

- FDA Approves New Antibiotic for Pneumonia, Skin Infections

October 30, 2010 — The US Food and Drug Administration (FDA) yesterday gave clinicians a much-needed new weapon against tough bacterial infections by approving ceftaroline fosamil (Teflaro; Forest Laboratories) for treating community-acquired bacterial pneumonia (CABP) and acute bacterial skin and skin-structure infections (ABSSSI), including the notorious methicillin-resistant Staphylococcus aureus (MRSA).

Ceftaroline, an injectable antibiotic, is a cephalosporin, which counter bacteria by interfering with their cell walls.

The drug's debut comes at a time when bacteria strains such as MRSA have gained resistance to existing antibiotics, leading to higher morbidity and mortality. MRSA, a common pathogen in ABSSSI, is resistant to not only methicillin, but also more common antibiotics such as amoxicillin and penicillin. Antibiotic resistance has complicated the treatment of CABP as well.

"These are serious and potentially life-threatening infections for which new treatment options are needed," Edward Cox, MD, MPH, director of the Office of Antimicrobial Products in the FDA's Center for Drug Evaluation and Research, said in a press release. "FDA is committed to facilitating new antibiotic drug development."

In September, the Anti-Infective Drugs Advisory Committee of the FDA unanimously endorsed ceftaroline as safe and effective for treating both CABP and ABSSSI, although some members felt that the evidence for the latter indication was less impressive.

Common adverse effects of the new antibiotic include diarrhea, nausea, and rash.

Clinicians should not prescribe it for patients with sensitivities to cephalosporin antibiotics.

Therefore, a research grant of € 5,000 can be awarded this year by the GSVD for basic or clinical research in veterinary dermatology. Ordinarily projects are expected to be of one to two years duration. Applicants are expected to propose a project of scientific merit that is applicable to veterinary dermatology. Results should be presented on a GSVD meeting. Grants are evaluated on scientific merit, feasibility, and usefulness.

Call for Abstracts for Autumn 2009 Meeting 7th 8th November, Marriot Hotel, Manchester.

Abstracts for free communications are warmly invited for the Autumn meeting of the BVDSG. Abstracts can describe clinical cases, applied studies or basic science in the field of comparative dermatology. All abstracts will be subject to peer review and will be considered on their scientific merits.

Abstracts should be no longer than 800 words and written in 12-point Times New Roman, Microsoft Word 2007 compatible. They should comprise an introduction, materials and methods, results, conclusion and essential references. They should also include the names and affiliations of the authors (with the presenting author indicated) and any possible conflict of interest.

The abstract should be sent by e-mail to the Proceedings Editor paul.coward@virgin.net at least 8 weeks prior to the meeting. Authors will be informed whether their abstracts have been accepted as soon as possible after submission. The author will be asked to give a presentation at the meeting that will last no more than 12 minutes followed by a brief period for questions.

If you are considering submitting an abstract but are uncertain how to approach this please contact one of the BVDSG committee and we will put you in touch with an experienced member who will be happy to advise you.

If your abstract is accepted you will receive free registration at the meeting.

Background: The University of Sydney?s Veterinary Teaching Hospitals, based in both Sydney and Camden, are specialist veterinary hospitals that conduct a range of veterinary services and general consultations to the wider community. They are recently refurbished teaching hospitals with state-of-the-art equipment and a full suite of specialist, imaging, pharmacy, and pathology services.

We are seeking an experienced small animal veterinarian with good general clinical skills, current interest and some experience in dermatology, and a desire to obtain specialist qualifications in veterinary dermatology. The appointee must possess excellent communication skills, the ability to work within a team and to supervise undergraduate veterinary students, and must hold a veterinary degree that is registrable in NSW, Australia. The successful applicant will undertake a fellowship training programme with the Australian College of Veterinary Scientists, under the supervision of Dr Linda Vogelnest.

The successful applicant will be appointed to a full-time position, shared between the two teaching hospitals, and will encompass at least four days per week in dermatology working directly with the supervisor, and up to one day per week in general small animal consulting. Some participation in weekend consulting and after- hours duty will be required. Completion of a research project (project ready to start: successful grant application), and externships in other dermatology centres and other related disciplines are required as part of the fellowship training programme.
